Nationally, about 40 percent of Medicare patients discharged from a hospital are referred for post-acute care, a category that includes home health, skilled nursing facilities, inpatient rehabilitation, and long-term acute care. Of those patients, about 20 percent are readmitted to the hospital within 30 days, often an indicator of ineffective and uncoordinated care post-discharge.
