In this episode of SOAP Notes with Dr. Jay LaBine, Jay sits down with Lynn Rosenbach, vice president of post-acute services at Banner Health and Chief Executive for Banner Home Care and Hospice. As November is recognized as National Hospice and Palliative Care month, Rosenbach discusses the importance of identifying hospice patients and how to assist with their transition into hospice care. Also discussed on this episode is how Banner Health has faired in its transition toward value-based care and how it has been able to provide non-clinical care coordination.

Rosenbach spearheads the strategy and development for Banner Health’s growing post-acute care continuum, including network development and overseeing the operation for the largest care continuum in the state of Arizona, and in Northern Colorado.  She has an extensive health care background in a variety of settings, from home health and behavioral health and managed care.
